About “Unfiled Questions”: When you create a Classic quiz, Canvas automatically copies its questions into a built-in bank called “Unfiled Questions” — this is Canvas behavior, not Holon. Holon also creates a separate question bank named after your quiz. If you don't want the duplicate, you can delete the “Unfiled Questions” bank in Canvas under Quizzes → ⋮ (top-right) → Manage Question Banks.
